Making a Claim for a Brace?
 
When making claims for a brace, the following information is required:
 

- completed claim form
- your doctor's note with diagnosis
- a detailed receipt from the provider including the type of brace, whether the brace is rigid or semi-rigid, the body part (ex. right ankle/left ankle), and the type of brace or name of brace
 

If any information is missing information will cause a delay in processing the claim.

Completed Extended Health Claim Forms with supporting documentation should be mailed to:
